OSF OnCall Connect

OSF OnCall Connect makes it easier for you to get the care and support you need between doctor appointments. Once enrolled in OSF OnCall Connect, your care team can support you on your health and wellness journey. This may include:

  • Digital resources and personalized education
  • 24/7 access to a digital care team
  • Regular screening and assessment of your needs to connect you to local resources
  • Technology and equipment at no cost to you to connect you to the care you need

OSF OnCall Advanced Care

Health care can be confusing and overwhelming at times. OSF OnCall Advanced Care provides personalized care to help people age 18 and older manage certain health conditions, which may put them at a higher risk of needing emergency care or hospitalization.

You may be eligible if you have health conditions requiring frequent doctor or emergency room visits.

Our team will determine if you're eligible or your provider will refer you. An enrollment specialist will then contact you to enroll.

Once enrolled in OSF OnCall Connect or OSF OnCall Advanced Care, the level of support you can expect will depend on the services you need.

Chronic condition monitoring

If you enroll in OSF OnCall Connect because you need monitoring of one of the following chronic conditions asthma, COPD, diabetes, heart failure or high blood pressure, you will receive 24/7 access to your care team. They will reach out by text message or phone call. You will receive outreach as needed that will allow you to share updates on your vitals and receive education, support and guidance about your condition, all from the comfort of your own home.

Health and wellness education

If you enroll in OSF OnCall Connect because you want health and wellness education concerning diet and nutrition, smoking cessation, exercise or weight management, you will receive education and support and be connected to resources in your community that can help you achieve better health.

Pregnancy care and postpartum support

If you enroll in OSF OnCall Connect because you are pregnant or want postpartum support, you will have 24/7 access to specially trained nurses and advanced practice providers. Through an easy-to-use app, you will receive information and education for each week of your pregnancy and for during your postpartum time.

OSF OnCall Advanced Care

If you are enrolled in OSF OnCall Advanced Care, you will have 24/7 access to your care team by phone or a tablet. Your personalized care and support is designed with your specific needs in mind and connects you to the right resources. You also will have access to other health care providers, such as social workers, pharmacists and dietitians, as needed.

You are eligible if you are age 18 and older, have Medicaid, and have one of the health conditions listed or need more support in managing your health and wellness. If you have an OSF provider, you will be contacted by one of our enrollment specialists by text message or phone call with instructions on how to enroll. If you do not have an OSF provider, you can get started through the enrollment links above, where you will receive further instructions.

This enrollment process should also be followed by any OSF Mission Partner and their family members covered under the OSF group medical plan who are age 18 and older and have one of the chronic conditions listed.

For pregnancy and postpartum care and support, you can self-enroll if you are a woman of any age in the state of Illinois who has Medicaid.

For OSF OnCall Advanced Care, eligible patients will be contacted by an enrollment specialist.
