OSF HealthCare has partnered with area elementary, junior high and high schools to offer a unique fundraising opportunity for your school athletics! For every athlete participating in the $25 sports physical clinic hosted at Illinois Valley Community College, OSF HealthCare will donate 100% of the proceeds back to benefit the students’ school!

Any Illinois Valley area student can complete their sports physical for the 2024-25 school year during this special on-site event. No appointments are needed for this walk-in clinic.

PLEASE BRING:

$25 cash or check, payable to OSF HealthCare (financial assistance is also available)
Completed consent and IHSA forms attached to this document

Note: A sports physical does not meet the requirement for an incoming freshman’s school physical. Parents should contact their child’s primary care doctor or pediatrician to schedule a school physical, as immunization records are required.
