Our team will make every effort to meet your needs. They may also answer questions you may have about what insurance plans are accepted.
Online: If you are already an OSF Medical Group patient and you don't have an urgent need, you may request or schedule an appointment online through OSF MyChart 24/7.
- Please arrive 15 minutes early to complete any registration paperwork that is needed.
- Co-payments are expected at the time of service.
- Minors must be accompanied by a parent or legal guardian.
- Please inform our staff when scheduling your appointment if the appointment is due to a work related injury or motor vehicle accident.
What Emergency Service Do I Use?
Wondering whether to use the Emergency Department, OSF PromptCare or OSF OnCall virtual urgent care clinic?
The is the best place for treating potentially severe and life-threatening conditions such as heart attack or stroke.
clinics can be used for urgent conditions that aren’t life-threatening but need to be taken care of right away, such as sprains, skin rashes or a severe sore throat.
OSF OnCall, a virtual urgent care clinic, can also be used for these urgent needs and has the added convenience of being accessible 24/7 from the home or office.
What to Bring
Make sure to bring your updated insurance card, a list of any medications you are currently taking, any payments or co-payments, and a photo ID to help us verify your identity and protect your privacy.
To assist us in making a smooth transition of care for you and your family, please arrange to have your medical records transferred to the office you will be visiting.
Forms
It may be necessary to complete a patient information or patient history form before your first appointment. Your doctor's office will give you instructions on what form you may need to bring. You may have also been asked to fill out an Adult Medical History form through OSF MyChart .

Cancellation Policy
If you need to cancel or reschedule your appointment for any reason, please provide us with 24-hour notice.
Failure to show for two (2) or more appointments without calling to cancel or reschedule may result in dismissal.
Referrals
Some doctors, especially specialists, may need a referral before a first appointment can be scheduled.
