Our transitional care program at Almost Home Kids provides a warm, home-like setting where your child will receive expert medical care while you and your family gain the knowledge and confidence needed for the next step.

What is transitional care?

Transitional care provides a short-term, supportive environment for your child with medical complexities when they’re ready to leave the hospital but not quite ready to be at home. Transitional care is like a bridge from a hospital stay to home.

Your Child’s Transitional Care Stay

At Almost Home Kids, your child stays in a comforting, home-like environment where they can grow, heal and thrive as you prepare to care for them at home. Our goal is to get your child home in 120 days.

You and your family are welcome to visit as often as you like and participate in daily care routines.

During their stay, your child will receive:

  • 24/7 medical support from skilled pediatric nurses and caregivers
  • Individualized care tailored to their unique medical needs
  • A safe, nurturing space that encourages growth and comfort

While your child receives expert medical attention, you and your family will get hands-on training to manage at-home medical care confidently.
